Sessions with the children I see with CAS include initially building trust and rapport. I work with parents and the child to select a list of target words that are functional, motivating, and will allow for the most progress such as words with different syllable shapes, vowels, and consonants. Usually I will start with 3 to 5 of those targets to work on, depending on the severity of the child's CAS. I keep data to determine when to remove a word from the session targets and add a new one. I generally do not have any toys or games down during practices, but I allow for breaks when needed. Initially, we practice saying the target words together with emphasis on the child watching my mouth and movements. I continue to fade the cues and feedback I give the child as they become more successful with the goal being the child producing the target word by themself. We usually will play with a toy or game of their choosing at the ending of their sessions to keep things fun and enjoyable for them. I try to incorporate target words they are able to produce spontaneously in the play, so they get additional practice with generalizing those targets.
I ask parents to help me come up with 'wish list' words for their child to help with functional targets. Parents are always welcome in the therapy room at their comfort level. I provide them with progress updates and explanations during or after the sessions.
I have used high tech AAC trialing and completed AAC evaluations for funding devices with children with low intelligibility who were frustrated with their difficulty communicating. I allow them to use their speech generating devices when they want to without ever forcing them or expecting them to use them. With other children I incorporate basic signs while we work on improving their speech.
