I use an eclectic approach to developing therapy goals and treatment strategies tailored to each child's needs and always in collaboration with families so as to best address their needs. With my youngest client's I try to target functional vocabulary, using a target word approach with words selected based with motor speech skills in mind , include parent training and coaching in a play and daily routine context and I also try to introduce gestures/modified signs/AAC to facilitate communication and reduce frustration. When children are a bit older, I also introduce a literacy approach- I use a programme based on a reading approach known as Words in Color where the kids do sound drills in sound combinations and quickly move to simple words and phrases incorporating these words. I determine whether strategies such as use of visual placement cues, tactile cues such as PROMPT approach or even kinesiotaping would be helpful and use a selection of approaches including but not limited to maximal pairs or other drill approaches. The speech targets are incorporated into games and other fun activities ,often chosen because of the child's expressed interests that are motivating and allow for lot's of repetition of sound targets. I like to start sessions with a brief conversation, move to therapy probes and more drill based activities and frequently end with a book reading/story telling.
Goals are always selected in collaboration, parent coaching in strategies and home activities which may be structured activities such as works sheets, online speech games or less formal ways to incorporate the child's speech targets into daily routines are provided.
I worked with the AAC specialists when I was at Holland Bloorview- they provided the training on high tech devices which I then could use with the clients if their speech was not adequate to meet their needs. I introduced low tech core vocabulary picture boards or theme boards and gestures or signs to facilitate communication and also worked on a variety of clarification strategies such as rate control, introducing topics and organizing information based on wh questions.
