Describe treatment approach used: I begin most sessions with a whole body sensory activity attuned to the child's sensory needs. This helps place the child in the an optimal state of sensory organization for improving learning. This is achieved using a variety of equipment such as swings, balls, balance beams and scooterboards which provide vestibular and proprioceptive input to either calm or arouse the child's sensory system.
I use an eclectic approach that incorporates PROMPT, multisensory cueing and DTTC. My therapy is overall guided by the principles of Motor Learning which means I try and see children with CAS 2 - 3 x per week for about 40 minutes. I believe I am highly skilled at getting motivation and participation from my patients and this comes from my Greenspan Floortime background as well as my studies with Pamela Marshalla. I structure my therapy sessions into pre practice, practice and review. I work to get high levels of repetition and pay close attention to my selected targets at every session. I work on variability and prosody early on in treatment I also am constantly evaluating my feedback depending upon what level a child is at. During prepractice I use more mass practice and then move towards a more distributed practice schedule. I believe my children are able to make great progress because through my tactile and proprioceptive cueing they are better able to create new motor plans. Then as we progress in therapy I do a dance as I like to say where I progressively remove cueing and help the child's motor system to take over. By following Motor speech principles I see my kids are able to generalize new motor speech sequences outside of the therapy room. I also work very closely with families to ensure carry over and home practice.
Describe parent/caregiver involvement: The majority of time parents sit in my sessions on a couch and observe. I always begin sessions by asking families how their child has been using their speech. I ask them to report if any new sounds have emerged or if any old sounds have become over habituated. I also like to know if any specific events occurred or if they have suggestions for words that they would like us to target. During sessions I comment to the parent about what cueing methods I am using and try to explain about the importance of feedback. I basically try and break down my process and work with the parent so they better understand why I am working on specific targets.
Describe your past and current involvement in the Apraxia community? I have organized support groups for families in the Hudson NJ area. I attend CASANA national conventions and am constantly reading the latest research as well as taking courses to expand my knowledge base in this area and stay up to date on the latest research.
