Depending on level of severity and age. Most of my clients are preschoolers to age 6. First I would obtain baselines of vowel, diphthong, and consonant productions as well as any CV, VC combinations elicited. Often, I use environmental sound imitation in play (me-ow, woof, woo-woo, whee, beep, etc.) with multiple repetitions, change of inflection, duration, loudness. Will then move environmental sounds into functional word sets for use in sessions. Ex. "oh-oh" may move to "open", "uh" to "up", expanding to "open up". Then use of a toy filled box to "open up" repeatedly. May add "I open", "open now", "may I open" depending on successes. "Me-ow" may move to "Me, out" "Me, in" with related play activity "put me in" "take me out". I often use "up" with building activities, starting with prolonged "uh" with plosive pop for /p/. As "up" is elicited in more natural productions, move to "up on", then "up on top". Randomly mixing with other simple VC, CV combinations to get "puppy up", "pop up" "down, up" or "up down". As more successes with vowel, diphthong and consonant imitations occur, more combinations for CV, VC, CVC, CVCV, VCVC etc. are used and often put into a self-made, personalized "book" for the child. Use of family pictures assist with target phrases: mom home, papa in home, daddy run, one more cookie, etc. Parents attend part or all of session and are expected to practice with me in functional play so they can reinforce and get productive practice in natural environments.
I attend Continuing Education events as much as possible. I support fund-raising efforts locally. I refer staff and parents to the Apraxia Kids website for resources.
Parents attend or observe sessions. They assist with development of targeted communication words/phrases relative to the family's needs. Parents are expected to reinforce successes from sessions at home in natural contexts. Therapists are expected to coach parents, provide ideas to make speech functional and fun at home.
