Treatment will begin by engaging the child with non-speech activities that involve imitating both big and small movements of the body to prepare him/her for therapy. Non-verbal cues such as eye contact, body orientation and maintaining attention will be targeted. Treatment will consist on developing a list of phonemes the child already has and what the child is stimulable for during the evaluation. Every session will focus on the principles of motor learning, which includes the accuracy of movement by frequent practice. Depending on the severity of CAS, a modified block approach that focuses on 1-2 items with more than 10 trials per block will be used while playing a fun game so that the child does not get bored. Movement sequences will be targeted as well as prosody early on. Treatment will consist of a combination of Dynamic Temporal and Tactile Cueing, Kaufman's workbook and PROMPT.
I am actively treating clients with CAS and have become passionate about educating other SLPs and caregivers about evaluation and assessment. I am looking forward to attending the advanced seminar that Dr. Edythe Strand hosts this year to gain more information and hopefully, become an expert.
Once CAS has been confirmed, parents are immediately provided an explanation of the disorder, specific techniques that will be used in therapy and any questions will be answered. Parents are encouraged to sit in on the sessions and learn DTTC techniques to carryover when the clinician is not there. Scheduling is of utmost importance since the initial treatment period consists of mass practice 4-5 days out of the week. Homework such as word lists and movement patterns to practice will be given after every session.
I have experience with LAMP, PECS, Coughdrop and many low-tech devices such as core boards and visual schedules. Children that have demonstrated significant severity in their ability to imitate syllable shapes and are older in age, require a communication device to have their wants and needs met as frustration may occur. AAC is an area I am very interested in and hope to attend more CEU's in the future.
