I start with whatever sounds, syllables or words that a child can produce either spontaneously or in imitation and then use that to build more syllable/word structures depending on the articulatory placement and manner. I then target words within a child’s daily routines to provide many opportunities for practice throughout the day. I use strategies from Dynamic Temporal and Tactile Cueing as well as PROMPT to elicit correct oral postures and sequencing of sounds into words. As soon as possible, I try to use the rule of 3 to get in as much practice on a target as possible and use knowledge of performance feedback to get the child to understand what they are doing well. As soon as possible, I try to fade the cues and use more knowledge of results as well as random practice for generalization as we move forward onto novel sounds and syllable structures. I have the family practice previously targeted words in a variety of routines for maintenance. For older children, I work on embedding target words into phrases and then sentences in an effort to generalize coarticulatory proficiency.
I am an active member in the Lehigh Valley Apraxia Network providing assistance to a parent support group along with speech-language pathologist Heather Hamilton. I help to organize and run the annual Apraxia Walk for Children. I co-organize and run a week long Apraxia Camp for Children each summer.
I have the parents of very young children practicing target words within a variety of routines throughout the day (play, snack, dressing, bathing, diaper changes/potty time, car rides, songs, book reading, etc). I teach them to use the rule of 3 to elicit as many responses as possible. I also teach the parent to position themselves so that the child can see their face and how to provide slow models either simultaneously or within delayed imitation. I also teach the parent to use gestural prompts and give the sounds relevant names to increase their reference and recall of the motor plan for those sounds. I usually construct a core vocabulary book for daily review with the parent for practice. We then add pictures as the child's sound/word skills improve. For preschool and older children, I provide worksheets with target words and try to develop games in which the child can have 10-15 minutes daily practice with the adult. I also encourage the parents to have structured dinner conversations with emphasis on the correct word productions during that time when possible. Book reading is also recommended to practice target words.
I will often use signs initially if the child is willing and able to use these to decrease frustration for communicating needs. I will use PECS or a core vocabulary book or chart to have the child point to wants and needs. I have also used the ProLoQuo2Go to supplement speech practice and to use for words/phrases that are not yet established.
